The Family Service Coordination services are carried out by a designated individual to assist and enable the families of children from birth to 3, to access, and if determined eligible, receive early intervention services. The Family Service Coordinator helps to develop the Individualized Family Service Plan (IFSP); assists the family in receiving all services identified; coordinates those services; ensures that they are delivered in a timely manner and seeks additional services and or supports that may help the child or family. These services will be conducted within a variety of environments in home and community settings.

Education and Licensure: A bachelor’s degree in social work; counseling; psychology, nursing; special education; early childhood education or closely related field.
Individuals with a bachelor’s degree in another field can substitute two (2) years of direct experience in serving individuals with disabilities and/or families.
